About the position

The LVN Case Manager at Emanate Health is responsible for coordinating care and services for selected patient populations, ensuring effective utilization and monitoring of healthcare resources. This role involves interaction with the Emergency Department and Registration Department, acting as an overseer during patient admissions to safeguard the interests of patients, the hospital, and physicians.

Responsibilities

Requirements

• Current California LVN License
• BLS Certification
• 2 years of recent acute clinical experience
• Experience with discharge planning, utilization management, and critical pathways preferred
• Effective written and verbal communication skills
• Strong time management skills
• Ability to work independently and as part of a team
• Knowledge of medical information systems
• Computer skills for making notations
• Excellent customer service skills

• Certification in Case Management preferred
