About the position

The Licensed Vocational Nurse (LVN) Case Manager II for the Every Woman Counts program plays a crucial role in a multi-disciplinary healthcare team, focusing on delivering comprehensive services to patients enrolled in the program. This position serves as the primary contact for patients and their families, assisting in the development and implementation of tailored healthcare plans to promote continuity of care and improve referrals, ultimately aiming to reduce the incidence and severity of breast and cervical cancer.

Responsibilities

  • Act as the primary contact for patients and their families enrolled in the Every Woman Counts program.
  • Assist the healthcare team in developing and implementing individualized healthcare plans.
  • Promote continuity of care and improve referral processes for patients.
  • Travel among clinics to provide case management services as needed.
  • Maintain cultural competency while interacting with diverse patient populations.
  • Utilize excellent organizational and problem-solving skills to prioritize tasks effectively.

Requirements

  • High school graduate or GED equivalent.
  • Completion of a state-approved vocational nursing program and current California nursing license.
  • Bilingual in English and Spanish with strong written and oral communication skills.
  • Ability to work independently and as part of a team.
  • Current CA Driver's License and valid vehicle liability insurance.
  • Willingness to travel among clinics for case management duties.
  • Cultural competency in healthcare settings.
  • Excellent organizational and problem-solving skills.
  • Basic computer skills including word processing and data entry.
  • Current CPR certification in compliance with corporate policy.

Nice-to-haves

  • Experience in practical nursing, preferably in an ambulatory patient facility.
  • Patient education expertise in areas such as pediatrics, family planning, prenatal care, diabetes, or chronic disease management.
