About the position

The LVN - Case Manager position at Sacramento Post Acute involves overseeing the daily functions of nursing assistants and ensuring compliance with established policies and procedures within the long-term care facility. In addition to administrative duties, the LVN will engage in direct patient care, including administering medications, performing assessments, and documenting care provided. The role also involves collaborating with physicians and other healthcare professionals to ensure that residents receive appropriate care and treatment. The LVN will be expected to participate in the development and implementation of care plans, monitor residents' conditions, and provide education and support to residents and their families. The position requires strong leadership skills, as the LVN will be responsible for evaluating staff performance, making staffing recommendations, and ensuring that nursing assistants are properly trained and supervised. The LVN will also play a key role in maintaining a safe and sanitary work environment, adhering to infection control protocols, and ensuring compliance with safety regulations. Responsibilities

  • Direct the day-to-day functions of nursing assistants in accordance with current rules, regulations, and guidelines.
  • Ensure compliance of nursing personnel with facility policies and procedures.
  • Participate in the development and implementation of the facility's quality assurance program.
  • Complete accident/incident reports as necessary and maintain the Daily Census Report.
  • Prepare and administer medications as ordered by the physician, ensuring proper verification of resident identity.
  • Participate in employee performance evaluations and staffing recommendations.
  • Ensure that nursing service personnel follow established safety regulations and infection control procedures.
  • Review care plans daily to ensure appropriate care is rendered and inform the Nurse Supervisor of necessary changes.
  • Monitor nursing procedures to ensure efficient use of supplies and equipment.
  • Participate in developing and conducting in-service training classes for nursing staff.

Requirements

  • Must possess a Nursing Degree from an accredited college or university or be a graduate of an approved LPN/LVN program.
  • Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
  • Current CPR certification is required.
  • Knowledge and experience with PCC preferred.
  • Ability to read, write, speak, and understand the English language.
  • Demonstrated knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

Nice-to-haves

  • Experience in long-term care facilities is preferred.
  • Familiarity with electronic health records (EHR) systems.
  • Bilingual skills may be beneficial for communication with residents and families.
