About the position

As a hospice nurse, you will play a crucial role in providing compassionate care to terminally ill patients. Your primary responsibilities will include developing individualized care plans, monitoring patients' conditions, and offering guidance and support to both patients and their families. You will work in various settings, including private homes, residential care facilities, and nursing centers, ensuring that patients receive the highest quality of care during their final stages of life. This position requires a deep understanding of end-of-life care challenges and the ability to provide emotional support to grieving families. You will be expected to collaborate with a multidisciplinary team, including physicians, social workers, and nursing assistants, to ensure comprehensive care for each patient. Your role will also involve maintaining accurate medical records and communicating any changes in patients' conditions to the appropriate healthcare providers. The ideal candidate will have experience in a hospice environment and a strong commitment to delivering exceptional end-of-life support.

Responsibilities

  • Collaborating on terminally ill patients' care with physicians, social workers, chaplains, nursing assistants, and other stakeholders.
  • Preparing and adjusting patient care plans as needed.
  • Frequently visiting patients to monitor vital signs and physical comfort levels.
  • Performing prescribed treatments and administering pain management medications.
  • Informing physicians about changes in patients' conditions.
  • Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
  • Maintaining and updating medical charts, patient files, and health records.
  • Notifying attending physicians, hospice staff members, and family members of patients' deaths when they occur.
  • Advising grieving family members on coping techniques, counseling services, and available community resources.

Requirements

  • State-approved LVN certification.
  • Competency in electronic patient and hospice management systems, such as HospiceMD, JNCLoud (Training will be provided).
  • In-depth knowledge of monitoring and managing terminally ill patients' conditions and comfort.
  • Superb analytical and diagnostic skills.
  • Great interpersonal, communication, and collaboration abilities, as well as a compassionate nature.
  • Exceptional ability to manage stress and emotional challenges.
  • Availability to work rotational shifts.
  • Availability to travel and drive to different locations within hospice area of coverage.

Nice-to-haves

  • Experience in a similar hospice environment.
