About the position

The Licensed Vocational Nurse (LVN) for Street Medicine at the Keck School of Medicine of USC plays a crucial role in providing care coordination and transitional primary care to unsheltered homeless individuals in Los Angeles. This full-time, in-person position emphasizes high-quality care for complex patients and collaboration within a multi-disciplinary team. The LVN will engage in direct patient care, documentation, and advocacy, ensuring that the needs of vulnerable populations are met effectively.

Responsibilities

  • Accurately obtains and records patient vitals and medical history.
  • Conducts follow-ups with patients via phone calls, encampment visits, and visits to other settings where patients can be found.
  • Recognizes changes in the patient's physical and mental condition, takes appropriate action by notifying physician and/or nursing supervisor.
  • Maintains files and records, oversees equipment and office supply inventory.
  • Provides comprehensive medical record documentation that reflects all aspects of care provided during each encounter.
  • Promotes security by alerting appropriate personnel for safety, fire and other unsafe situations.
  • Demonstrates knowledge and skill necessary to provide individualized interventions and services to promote restoration of health and achievement of patient health goals.
  • Provides client care coordination in a manner that is non-discriminatory and non-judgmental.
  • Acts as a patient advocate and liaison between the patient and community agencies.
  • Works closely with Street Medicine Registered Nurses, Nursing Supervisor and Clinical Coordinator, Physician/Physician Assistant/Nurse Practitioner, and several team members of the multi-disciplinary team.

Requirements

  • High school diploma or equivalent.
  • Completion of an accredited vocational nursing program required.
  • Must have passed the NCLEX-PN licensing examination.
  • Valid California Vocational Nursing license.
  • BLS for Healthcare Provider certification.

Nice-to-haves

  • 1 year experience with unhoused population.
  • Demonstrated intrapersonal skills and knowledge of patient care and/or needs.
  • Venipuncture certification.
