About the position

The Licensed Practical/Vocational Nurse (LPN/LVN) at WellMed, part of the Optum family, plays a crucial role in supporting patients with stable chronic health conditions. This position involves navigating the healthcare system, empowering patients through education and advocacy, and collaborating with multidisciplinary care teams to enhance patient care and quality of life. The LPN/LVN will work in a self-directed environment, performing nursing duties within their licensed scope, and contributing to the overall mission of improving healthcare for seniors.

Responsibilities

  • Identify patients with chronic disease diagnoses in collaboration with providers and clinic staff.
  • Support longitudinal care of patients with stable chronic conditions through focused assessments and medication reconciliation.
  • Conduct Motivational Interviewing and assist in self-management goal setting.
  • Provide patient education using standardized materials and create referrals to appropriate resources.
  • Facilitate the transition of patients from inpatient to outpatient settings by assessing transitional needs and reviewing contingency plans.
  • Notify providers or supervising RN of any changes in patients' health conditions.
  • Document patient interactions accurately and timely in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare reports for weekly meetings as required.
  • Maintain continued competence in nursing practice and stay updated on evidence-based practices.

Requirements

  • Active and unrestricted Licensed Practical/Vocational Nurse license in the state of employment.
  • Current BLS certification.
  • Minimum of 2 years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases such as COPD/asthma, diabetes, CHF, and IHD.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills and ability to work with multidisciplinary teams.
  • Proficient computer skills for electronic medical records management.
  • Excellent organizational and prioritization skills.

Nice-to-haves

  • IV Certification
  • Experience in patient education and motivational interviewing skills.
  • Fluent in English and Spanish.
