About the position

We are seeking a compassionate and dedicated Home Health and Hospice Nurse to provide high-quality care to patients. The ideal candidate will possess a strong understanding of Home Health and palliative care principles and demonstrate excellent communication skills to support both patients and their families. This role requires a commitment to maintaining patient dignity and comfort while ensuring compliance with healthcare regulations.

Responsibilities

• Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
• Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
• Provide acute pain management strategies to ensure patient comfort.
• Conduct thorough triage assessments to prioritize patient care needs.
• Collaborate with multidisciplinary teams for discharge planning and continuity of care.
• Educate patients and families on hospice services, symptom management, and end-of-life issues.
• Maintain accurate documentation in compliance with HIPAA regulations using electronic health records systems such as Cerner.
• Offer emotional support and counseling to patients and families during difficult times.

Requirements

• Valid nursing license in the state of practice.
• Experience in inpatient settings, particularly within hospice or palliative care environments.
• Familiarity with discharge planning processes and acute pain management techniques.
• Knowledge of pediatric care, including toddler care, is a plus.
• Proficiency in electronic health record systems; experience with Cerner is preferred.
• Strong understanding of HIPAA regulations to ensure patient confidentiality.
• Ability to work collaboratively within a healthcare team while demonstrating empathy and compassion towards patients and families.

Nice-to-haves

• Previous experience in ophthalmology or PICU settings is advantageous but not mandatory.
