About the position

The Licensed Vocational Nurse (LVN) position at Epic Home Health Care involves providing in-home care to patients in the San Gabriel Valley area. The position is part-time and per diem, allowing for flexible scheduling while supporting the needs of the senior population.

Responsibilities

• Provide in-home care to patients according to the Plan of Care.
• Conduct patient assessments and monitor vital signs.
• Perform catheter care and tube feeding as needed.
• Document patient care and observations in compliance with state and federal guidelines.
• Communicate effectively with patients, families, and healthcare professionals.
• Participate in staff meetings and in-service reviews.

Requirements

• Current LVN License in the state of CA.
• Dependable transportation and a valid Driver's License with auto insurance.
• Current CPR certification.
• Ability to perform the physical aspects of the position.
• TB testing and annual flu vaccinations.

Nice-to-haves

• Home health experience (preferred but not necessary).
• Experience with EHR and EMR systems.
• Strong organizational skills.
