About the position
The Licensed Vocational Nurse (LVN) position at Doctor's Choice Home Care & Hospice involves providing skilled nursing care to hospice patients in their homes. The role requires adherence to physician orders and collaboration with Registered Nurses to develop and evaluate care plans. The LVN will also engage in patient and family education, communicate with healthcare providers, and participate in quality assurance programs.
Responsibilities
- Administer hospice skilled nursing care to patients in their homes.
- Develop and evaluate the plan of care in partnership with patients and caregivers.
- Participate in patient, caregiver, and family counseling and education.
- Communicate with physicians and healthcare practitioners regarding the plan of care.
- Provide services as ordered by the physician in accordance with the plan of care.
- Contribute to all phases of the QAPI program and participate in interdisciplinary assessments of patients.
Requirements
- Currently licensed as an LVN in the state of practice.
- Minimum of three years of nursing experience, specifically in Home Health.
- CPR certified by the American Heart Association or American Red Cross.
- Strong communication skills, both oral and written.
- Ability to exercise initiative and independent judgment.
- Valid driver's license and reliable transportation.
