About the position

Responsibilities

• Provide nursing care under the supervision of a Registered Nurse or physician.
• Demonstrate proficiency in nursing skills and competencies.
• Uphold Kaiser Permanente's policies and procedures.
• Utilize effective communication strategies to achieve intended outcomes.
• Participate in problem identification and resolution.
• Mentor and orient others in skill and behavioral learning.
• Prioritize and delegate work assignments appropriately.
• Perform patient care activities considering age-related needs.
• Report unusual findings to the Registered Nurse and contribute to care plans.
• Collaborate with the healthcare team to develop and monitor individualized care plans.
• Document patient care activities accurately and legibly.
• Maintain a safe environment and apply standard precautions.
• Discuss patient findings and progress with the healthcare team.
• Practice customer service standards and maintain patient confidentiality.
• Act as a resource and mentor to new employees and team members.

Requirements

• Minimum six months acute care and/or related outpatient experience within the past four years.
• High School Diploma or GED.
• Vocational Nurse License (California).
• Basic Life Support certification.
• I.V. Certification updated within the last 3 years or demonstrated I.V. competencies.

Nice-to-haves

• Experience in a Home Health/Hospice Department.
• Ability to read and comprehend simple instructions and correspondence.
• Ability to write simple correspondence.
• Ability to effectively present information in one-on-one and small group situations.
