About the position

The Licensed Vocational Nurse (LVN) at ConcertoCare plays a crucial role in delivering clinical care to patients in their homes and providing virtual clinical support. This hybrid position involves both in-home visits and virtual care facilitation, ensuring that patients receive timely and appropriate care. The LVN is a key member of the care team, responsible for direct patient care, health screenings, and care coordination, all while adhering to compliance policies and maintaining accurate documentation.

Responsibilities

• Provide direct nursing care to patients through routine and urgent in-home and virtual visits under the direction of an authorized health care practitioner.
• Facilitate virtual visits in the patient's home to support care delivery under the direction of a remote clinical practitioner.
• Provide support for clinical visits, including chart preparation, clinical intake, after-visit care coordination, and clinical documentation and order entry support.
• Coordinate and support patient care by implementing care plans and performing outbound calls and tasks as directed by a clinical supervisor.
• Process and track orders and referrals placed by providers, ensuring patients complete them according to their treatment plan.
• Engage patients in their healthcare journey, establishing meaningful connections and providing tailored care.
• Deliver exceptional customer service to patients, caregivers, and clinical partners with proactive communication.
• Perform clinical skills and procedures such as blood draws, medication administration, wound care, and IV insertion under supervision.
• Document all patient care support activities accurately and timely in the medical record.
• Participate actively in team huddles and organizational meetings as requested.
• Adhere to compliance policies, procedures, and standards of conduct.

Requirements

• Hold an active unrestricted LVN license in the state of California.
• 2+ years clinical experience required, with urgent care, emergency room, or community home health experience preferred.
• IV or blood draw certification or relevant work experience.
• Current certification in Basic Life Support (BLS) required.
• Ability to conduct independent patient home visits.
• Ability to travel 75%+ of the time within an assigned area.
• Average to advanced computer and software skills required.
• Proof of liability and property damage insurance on the vehicle used is required.
