About the position

The Licensed Vocational Nurse (LVN) Reward PRN is responsible for delivering patient care as part of a healthcare team, utilizing the BSWH nursing professional practice model. This role involves conducting focused assessments, participating in care planning, and documenting patient conditions in accordance with the Texas Nursing Practice Act. The LVN is expected to create a supportive environment for patients and families while collaborating with RNs and other healthcare professionals to ensure quality care.

Responsibilities

• Conduct accurate focused assessments according to practice standards and LVN Scope of Practice.
• Report data to assist RN in developing a plan of care and formulating goals/outcomes for patient and family needs.
• Evaluate the quality and effectiveness of nursing practice, including nursing sensitive indicators.
• Create a compassionate, supportive, safe, and therapeutic environment for patients, families, and staff.
• Manage emotional and physical pain to promote comfort and healing.
• Incorporate a patient's and family's unique differences into an individualized plan of care.
• Preserve and protect the confidentiality, autonomy, dignity, and rights of patients and families.
• Facilitate learning for patients, families, and nursing staff throughout the delivery of care.
• Collaborate with colleagues and community to implement an integrated plan of care.
• Participate in problem-solving strategies for patients, families, and staff.

Requirements

• Graduate of an accredited nursing program.
• 2 years of experience as a Licensed Vocational Nurse (LVN).
• Current License Pract/Vocational Nurse (LVN).
• Basic Life Support (BLS) certification or obtain within 30 days of hire.
