About the position

The Medical Assistant (MA) Care Coordinator for Refugee Health Services plays a crucial role in supporting newly arrived refugees and other eligible groups by facilitating their navigation through the healthcare system. This position collaborates with clinical teams to ensure patients receive necessary care and treatment, addressing barriers to healthcare access and providing comprehensive support throughout their care journey.

Responsibilities

  • Serve as a liaison for newly arrived refugees between HCPH Refugee Health Screening program and local resettlement agencies, social services, and healthcare providers.
  • Monitor, track, schedule, and follow up on abnormal test results, missed appointments, and other patient needs.
  • Coordinate patient referrals to services within Harris County Public Health or to community specialists.
  • Research and maintain community resources to support patients' overall care needs.
  • Document the patient's care journey as they complete medical and social services recommended for their care needs.
  • Schedule transportation and interpretation services for patients to clinic appointments.
  • Conduct home visits when necessary to retrieve medical records, obtain consents, or collect specimens.
  • Provide in-room support for clinical staff during clinic visits, including performing capillary and venous draws, urine collection, and vital signs.

Requirements

  • High school diploma or equivalent.
  • Completion of a community college or business/technical school Medical Assistant program.
  • Valid TX Driver's License.
  • American Heart Association CPR Certification.
  • Four (4) years of experience in a healthcare setting.
  • One (1) year of experience in care coordination.
  • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook).
  • Excellent communication skills (verbal & written).
  • Ability to establish and maintain effective working relationships in a diverse environment.

Nice-to-haves

  • Medical Assistant Certification.
  • Bilingual (English/Spanish).
  • Experience working in refugee and/or global health settings.
  • Experience in phlebotomy.
