About the position

As a Community Case Manager, you'll play a key role in empowering children, teens, and families by delivering individualized support to clients in homes, schools, and community settings. You will collaborate closely with licensed therapists to provide comprehensive care, lead engaging group sessions that promote social, emotional, and behavioral growth, and act as an advocate for families, connecting them to critical community resources. This position offers flexible hours, including some evening availability to accommodate client needs. Comprehensive training, ongoing mentorship, and a supportive team environment will help you excel in this impactful role.

Responsibilities

  • Supporting children and teens in achieving their treatment goals through personalized interventions.
  • Facilitating skill-building group sessions on topics like social skills, conflict resolution, and emotion regulation.
  • Providing education and advocacy to help clients navigate community systems effectively.
  • Assisting children and families in developing resilience and coping strategies for success in school, at home, and in their communities.
  • Maintaining detailed, accurate records to ensure quality care and compliance.

Requirements

  • At least 21 years of age.
  • A high school diploma or GED.
  • Strong organizational and problem-solving skills to work independently across multiple systems.
  • A valid driver's license, reliable transportation, and appropriate insurance.
  • Ability to pass a background check.
