About the position

The PACE Center Manager at PACE Baton Rouge is responsible for overseeing the effective operations of the PACE Center, ensuring that all participant services are coordinated and supervised under the general supervision of the Director. This role is crucial in providing an alternative to long-term care for seniors, allowing them to live with dignity in their communities. The manager will work closely with a multi-skilled team, including doctors, nurses, social workers, and therapists, to develop individual care plans and monitor the progress of participants. The manager will also be involved in the enrollment processes and the development of the Interdisciplinary Team (IDT) Plan of Care, ensuring that all activities related to new enrollees are efficiently coordinated. In addition to participant care coordination, the PACE Center Manager is responsible for ensuring compliance with established CMS/DHH regulations and standards. This includes understanding PACE regulations and ensuring that all staff are informed and compliant. The manager will oversee human resource management for various disciplines, including nursing and therapy staff, and will monitor staffing patterns to meet the agency's responsibilities. The role also involves working closely with the Director to develop and monitor contract services, ensuring quality service delivery. Leadership is a key component of this position, as the manager will serve as a liaison between different program disciplines, PACE departments, and community service providers. Effective communication between clinical and non-clinical staff is essential to provide the best care for participants. The manager will also participate in the management of the PACE budget and will be responsible for the administration of the electronic medical record, serving as a Super User for the EMR system.

Responsibilities

  • Coordinate all center activities including care planning and implementation preparation for Interdisciplinary Conferences and daily meetings, family conferences and all related follow-ups.
  • Coordinate all activities relating to new enrollees.
  • Communicate and coordinate all participant changes to team members.
  • Participate in the development of the IDT Plan of Care and is a part of the enrollment processes.
  • Participate in the coordination of a 24 hour care delivery and manage and participate in the 24/7 On Call Process.
  • Ensure that established CMS/DHH regulations and standards are met.
  • Understand PACE Regulations and Level 11 Reporting and ensure that PACE staff understands the Regulations.
  • Responsible for human resource management of all disciplines including RNs, LPNs, Therapists, Dietary, Pharmacy Staff, Clerical staff, Adult Care Assistants, Recreational Therapy staff, MSWs and Transportation personnel.
  • Monitor staffing patterns and determine the number, types of staff and supervision required to meet the agency's responsibilities.
  • Work closely with the Director in the development and ongoing monitoring of contract services including selection of providers and ongoing evaluation of quality of services.
  • Responsible for the Participant Advisory Committee as required by standards, ensuring quarterly meetings take place and coordinating the selection of committee members and chairpersons.
  • Responsible for the coordination of grievances according to standards and meeting the time frames required.
  • Serve as a liaison between individual program disciplines, other PACE departments and community service providers.
  • Implement effective communication between clinical staff, non-clinical staff and medical staff to provide the best of care and services to the Participants.
  • Participate in the Management team of PACE and effectively communicate any changes, new standards and rationale to the staff.
  • Participate in the Management of the PACE Budget.
  • Responsible for knowledge and administration of the electronic medical record and serve as a Super User for the EMR.

Requirements

  • Five years nursing experience with at least two years supervisory experience and one year caring for the frail elderly.
  • Graduate of Accredited RN School.
  • Able to function well in a team environment.
  • Possess good organizational, communication and supervisory skills.
  • Able to effectively handle difficult and unusual interpersonal situations.
