About the position
The Program Manager for the IHH Team at East Bay Community Action Program (EBCAP) will lead and direct the development of health services plans, monitor consumer status, and provide feedback on staff performance. This role involves supervising the health home team and guiding wellness initiatives and health education groups.
Responsibilities
- Provide leadership and direction for health services plans during treatment plan meetings.
- Monitor each consumer's status and response to health coordination and prevention activities.
- Provide feedback regarding staff performance.
- Supervise members of the health home team in developing wellness and prevention initiatives.
- Guide health education groups.
Requirements
- Master's Degree in Human Services with LICSW/LMHC certification.
- Three to five years of post-graduate clinical experience in rehabilitative mental health.
- 1-2 years of managerial and supervisory experience.
- Familiarity with RIACT2 regulations is a plus.
