About the position

The Care Coordination Manager - Partner/Satellite Services at Metrocare Services is responsible for overseeing community partner outreach and ensuring a seamless transition for individuals entering services. This role involves developing a vision for community engagement, coordinating referrals, and implementing a person-centered approach to care. The manager will work closely with the Care Coordination Team to provide trauma-informed support to adults and families, ensuring continuity of care and effective communication with community partners.

Responsibilities

  • Establish community engagement goals and objectives in collaboration with the Director of Care Coordination.
  • Build partnerships and relationships with organizations throughout Dallas County and the state.
  • Represent Metrocare at coalition and community group meetings and events.
  • Engage the public to inform them about Metrocare's programs and supports.
  • Support the Director of Care Coordination and Clinical Managers with assigned duties.
  • Assist with clinical and administrative supervision of the Care Coordination team as needed.
  • Receive and review community referrals, assigning them to appropriate staff.
  • Interface with other site managers to ensure program efficiency.
  • Facilitate scheduling and operations of co-locations with partners.
  • Ensure individuals receive follow-up services as recommended by their level of care.
  • Document services in the electronic health record within 24 hours after services have occurred.

Requirements

  • Bachelor's degree in psychology, social work, or a related human service field; Master's preferred.
  • 4-5 years of mental health experience.
  • Minimum of 3 years of management/supervisory experience.
  • Basic math skills for reporting activities and budgets.
  • Current State of Texas Driver License or equivalent.
