The position involves overseeing the day-to-day operations of the department, ensuring effective resource management for patient care delivery across the care continuum for assigned patient populations. The role requires being a positive role model for staff, coaching, educating, and supporting both employees and organizational growth.

Responsibilities

  • Mentor others in resolving issues and concerns regarding case management processes.
  • Assist leadership with managerial activities such as payroll, employee schedules, and educating employees.
  • Provide direct clinically based case management to an assigned group of patients when required.
  • Assist with providing support and education to the case management team through collaboration with appropriate case management and utilization resources.
  • Utilize the nursing process to provide individualized nursing care to patients.
  • Collaborate with inter-professional colleagues, including physicians, to plan, implement and evaluate care.
  • Plan and coordinate care, patient assessment, patient, caregivers, and family education, triage, and various other nursing interventions.
  • Function within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules.

Requirements

  • Bachelor’s degree in nursing required.
  • Graduate from a nationally accredited nursing program required.
  • Minimum of three years of clinical nursing experience required.
  • At least two years of case management/care coordination experience required.
  • Currently holds an unencumbered RN license with the State Board of Nursing.
  • Obtains and maintains required department specific competencies and certifications within 36 months.
