About the position

The Case Management Manager will oversee the planning, organization, and direction of case management services, ensuring quality and continuity of care. This role involves collaboration with healthcare professionals to improve resource utilization and provide supervision and feedback to case management staff. The manager will also be responsible for evaluating clinical and financial outcomes, as well as ensuring compliance with regulatory requirements.

Responsibilities

  • Oversee the planning, organization, and evaluation of care provided by Case Managers in collaboration with physicians and health professionals.
  • Stay updated on current case management concepts, utilization review principles, and discharge planning criteria.
  • Revise department procedures and processes in compliance with regulatory requirements in collaboration with the Director of Case Management.
  • Apply team management concepts to facilitate effective patient care coordination.
  • Direct and supervise care provided by case managers.
  • Develop schedules and assignments for case managers.
  • Organize, plan, and prioritize the work of Case Management Staff.
  • Assess educational needs of staff and develop appropriate educational programs.
  • Establish and evaluate clinical and financial patient outcomes with the Director of Case Management to maintain quality care.
  • Lead collaboration with physicians and multidisciplinary teams in developing and implementing critical pathways.
  • Guide staff in providing consistent information about disease processes and nursing prognosis through education.

Requirements

  • Bachelor's Degree in nursing or a related field or commensurate experience and education required.
  • Current California Registered Nurse licensure.

Nice-to-haves

  • Master's Degree in nursing or a related field preferred.
