About the position

The Manager of CCBHC Implementation at Harbor is responsible for overseeing the implementation of the Certified Community Behavioral Health Clinic (CCBHC) model across the agency. This role involves coordinating programmatic changes to meet certification criteria, monitoring Clinical Quality Measures (CQMs), and providing clinical supervision and training. The manager will also handle population health management reports and collaborate with team members for analysis and stratification.

Responsibilities

  • Provide ongoing coordination and oversight of the CCBHC implementation across the agency.
  • Monitor and report on required Clinical Quality Measures (CQMs).
  • Assist with clinical supervision and agency training needs related to model fidelity.
  • Run population health management reports and collaborate with team members for analysis and stratification.
  • Exhibit leadership and team-building skills to promote a positive culture among the team and across the agency.
  • Operationalize and implement the CCBHC model of care across the agency.
  • Track data and outcomes for population health management and CCBHC CQMs.
  • Lead and supervise assigned team members to ensure productivity and effectiveness in client care.
  • Assist in training for new clinical staff and agency-wide CCBHC trainings.
  • Coordinate with clinical leaders and Physician Led Patient Centered Teams for service coverage and crisis support.
  • Manage personnel functions for assigned staff, including supervision and evaluations.
  • Participate in innovation, development, implementation, and evaluation of services, including outcome and grant reporting.
  • Engage in community meetings and workgroups to enhance services and resources.
  • Participate in staff meetings and continuing education opportunities.

Requirements

  • Master's degree and license in behavioral health (LSW, LPC, LISW, LPCC) required.
  • Independent license in behavioral health (LISW, LPCC) preferred.
  • Excellent leadership and communication skills.
  • Ability to work successfully with interdisciplinary teams.
  • Valid driver's license and acceptable driving record.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
  • Health Officer certification required within 6 months of employment.
