About the position

The Manager, Community Health is responsible for supporting and executing The Health Collaborative's community health strategy in alignment with a regional Collective Health Agenda. This role involves developing and maintaining partnerships with community organizations, managing multi-stakeholder initiatives, and ensuring effective program management to promote health equity and improve community health outcomes.

Responsibilities

  • Develop and maintain sustainable relationships with community partners including neighborhood leaders, Community Health Centers, Hospitals and Health Systems, Community Councils, Local Health Departments, and Community Based Organizations.
  • Manage a Community Partnership Network to support the Regional Collective Health Agenda.
  • Serve as a liaison for THC to the community in local meetings.
  • Work closely with cross-functional teams to align projects and activities with organizational goals.
  • Support and grow relationships for key committees of Community Health pillar initiatives.
  • Track and monitor community partnerships in a CRM (e.g., Salesforce).
  • Oversee complex multi-stakeholder initiatives including the Community Health Needs Assessment (CHNA) and the Community Health Improvement Plan (CHIP).
  • Assist in identifying and crafting submissions for funding opportunities.
  • Develop project work plans and timelines to ensure team-wide transparency.
  • Secure and set up meeting spaces, manage invitations, attendance, notes, action items, and follow up.
  • Monitor and evaluate effectiveness of partnerships and initiatives through feedback and data collection.

Requirements

  • Bachelor's degree in public health, social work, healthcare administration, or a related field; Master's degree preferred.
  • At least 5 years' experience in a similar role.
  • Experience in managing partnerships with multiple organizations.
  • Strong leadership and interpersonal skills.
  • Excellent writing, presentation, and interpersonal communications skills.
  • Ability to prioritize, organize, manage, and fulfill expectations pertaining to multiple work streams.
  • Understanding of the healthcare system, public health infrastructure, and social determinants of health.
  • Proven experience managing projects.
  • Familiarity with data visualization and ability to interpret data.

  • Familiarity with collaborative technology platforms (Salesforce, Monday.com, Teams).
