The Manager of Community Integration Programs is a key member of the AbleLight team that delivers person-centered care and services to those we support in a supported living setting to fulfill our purpose for existing: “We believe the world shines brighter when people with developmental disabilities achieve their full potential.” This role focuses on facilitating the planning, development, and implementation of person-centered plans, ensuring that the needs and desires of individuals with developmental disabilities are met through tailored support and services. The Manager will be responsible for conducting assessments and evaluations, regularly reviewing data, and assimilating information into individualized support plans. This position requires a proactive approach to assist individuals with Intellectual Disabilities in gaining access to necessary medical, social, educational, and other services. The Manager will provide or coordinate direct case management activities, including needs assessments, case planning, service arrangements, social support, and ongoing monitoring of progress towards meeting established outcomes. Additionally, the Manager will attend interdisciplinary and interagency meetings as required and complete necessary documentation for accountability and billing purposes.

Responsibilities

  • Facilitates the planning, development, and implementation of person-centered plans by completing assessments and evaluations.
  • Assists individuals with Intellectual Disabilities in gaining access to needed medical, social, educational, and other services.
  • Provides or coordinates direct case management activities, including needs assessment and case planning.
  • Assists individuals in identifying necessary services and supports and linking them with needed resources.
  • Expands or establishes a social support network for individuals and families through linkage with appropriate persons and agencies.
  • Evaluates progress toward accomplishing objectives specified in the Plan at quarterly intervals.
  • Documents objectives, progress, and participates in any necessary revisions to the Plan of Care.
  • Attends and participates in interdisciplinary and interagency meetings as required.
  • Completes necessary documentation for maintenance of the individual's case management/service coordination record.

Requirements

  • Must be 18 years of age or older.
  • Bachelor's degree in a related field or commensurate experience.
  • Four years of case management experience.
  • Possession of a valid driver's license and good driving record.
