About the position

The position involves managing programs and leading clinicians who provide primary care to SCAN members within an integrated care delivery model. The role focuses on ensuring high-quality patient care, overseeing clinical teams, and developing new clinical products and services for seniors. It requires strong leadership, strategic thinking, and collaboration with senior management to enhance operational performance and achieve defined clinical care outcomes.

Responsibilities

  • Manage programs and clinicians providing primary care to SCAN members within the integrated care delivery model.
  • Oversee the performance of clinical teams to achieve defined clinical care outcomes.
  • Ensure adherence to ethics and quality standards in patient care delivery by Advanced Practice Providers (AAP).
  • Select, onboard, train, and develop clinicians within the integrated care delivery model.
  • Communicate job expectations and appraise job results; coach and counsel employees.
  • Provide clinical care including exams, diagnostic tests, and patient education.
  • Collaborate with senior management on operational and performance issues.
  • Develop and implement APP Best Practice Models across specialties and departments.
  • Monitor success metrics and adjust programs to improve efficiencies and effectiveness.
  • Network and build relationships with internal and external stakeholders to promote membership growth.

Requirements

  • Master's degree from an accredited school of nursing.
  • California RN, NP, and Furnishing licensure required.
  • Certification in area of specialty required.
  • 5+ years of recent NP experience required.
  • 3+ years supervisory experience preferred.
  • Demonstrated experience in working with both Medicaid and Medicare.
  • Strong strategic thinking skills for developing new clinical products and services.
  • Strong interpersonal, written, and oral communication skills; commitment to detail.
  • Demonstrated leadership and management skills.

Nice-to-haves

  • Experience in integrated care delivery models.
  • Familiarity with senior healthcare issues and solutions.
