About the position

The Manager of Home Care at Wesley Community Health Services is responsible for overseeing home care services for an assigned territory, ensuring that residents receive appropriate care and that home care aides deliver safe and effective services. This role involves direct supervision, client assessments, care plan development, and collaboration with other healthcare professionals to enhance service delivery and grow the volume of residents served in private homes.

Responsibilities

  • Supervise home care services and ensure compliance with company policies and state regulations.
  • Respond to inquiries for home care services and screen for appropriate levels of care.
  • Complete the admission process, including obtaining signed consent and financial agreements.
  • Conduct client assessments and develop individualized care plans.
  • Facilitate the initiation of services, including caregiver orientation and scheduling.
  • Maintain complete and timely client records in the electronic medical record (EMR).
  • Conduct ongoing evaluations of home care services and client reassessments every six months or as needed.
  • Hire, orient, train, and evaluate home care aides' performance.
  • Oversee care for approximately 30 home care clients and manage scheduling activities.
  • Participate in marketing activities to promote home care services and meet growth targets.

Requirements

  • High school diploma or GED required; Bachelor's degree in Marketing, Business Administration, Social Services or related field preferred.
  • Valid CPR and First Aid certification.
  • Valid Washington State Driver's License and active automobile insurance.
  • Washington State Certified Nursing Assistant license is a plus.
  • Minimum two years' experience in Home Care or similar supervisory or management role.
  • Demonstrated proficiency in providing direct supervision of caregiver staff.
  • Competence in technology use, including data entry into EMR and proficiency in MS Word and Excel.
  • Superior written, verbal, and interpersonal communication skills.

Nice-to-haves

  • Experience in back-office operations including scheduling, data management, and reporting.
  • Demonstrated interest and/or experience interacting with older adults and their family members.
