About the position

The Social Worker position at Fresenius Kidney Care involves providing psychosocial services to patients undergoing dialysis treatment, both in-center and at home. This entry-level role focuses on assessing patients' psychosocial needs, collaborating with healthcare teams to enhance patient care, and supporting patients and their families in understanding the emotional and psychological impacts of Chronic Kidney Disease. The Social Worker will also participate in quality improvement activities and patient education to ensure comprehensive care and improved quality of life for patients.

Responsibilities

  • Assess patients' psychosocial status and needs as part of the comprehensive patient assessment.
  • Participate in care planning with the healthcare team to identify effective interventions for rehabilitation and treatment goals.
  • Utilize patient education programs and social work methods to address barriers and meet treatment goals.
  • Provide supportive counseling services to patients within the scope of clinical training and state license.
  • Assist patients with referrals to community resources to facilitate optimal treatment outcomes.
  • Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with referrals.
  • Facilitate discussions regarding advance directives and DNR status with patients and families.
  • Document interactions and interventions provided to patients and families based on MSW policies.
  • Participate in monthly quality review meetings and report on quality indicators related to patient adherence and quality of life trends.
  • Provide education tailored to patients' learning styles and needs regarding kidney disease and treatment modalities.
  • Facilitate the transplant referral process and manage transplant waitlist in collaboration with the interdisciplinary team.
  • Review patient rights and responsibilities with patients and their representatives.
  • Support the clinic in scheduling to maximize patient adherence and adjustment during the admission process.
  • Collaborate with Insurance Coordinators to address insurance-related issues and provide education on payment options for dialysis treatment.
  • Assist with the hiring process of new personnel if requested and maintain updated patient resource lists.
  • Provide training to staff on psychosocial topics as needed and participate in weekly team huddles.

Requirements

  • Masters in Social Work
  • State required license
