About the position

This entry-level Master of Social Work (MSW) position provides psychosocial services to patients undergoing treatment at the facility, including in-center and home dialysis patients. The role involves collaborating with the healthcare team to enhance patient rehabilitation, quality of life, and emotional well-being, while also educating staff on the psychosocial impacts of Chronic Kidney Disease. The social worker will actively participate in quality improvement activities and support the organization's commitment to patient satisfaction and quality care.

Responsibilities

  • Assess patients' psychosocial status and needs as part of the comprehensive patient assessment.
  • Participate in care planning with the patient and healthcare team to identify effective interventions.
  • Utilize patient education programs and social work methods to address barriers and meet treatment goals.
  • Provide monitoring and interventions to help patients adjust to dialysis and improve their psychosocial status.
  • Offer supportive counseling services to patients within the scope of clinical training and state license.
  • Assist patients seeking transplant with educational and goal-directed counseling.
  • Provide information and referrals to community resources to facilitate optimal treatment outcomes.
  • Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with referrals.
  • Facilitate discussions regarding advance directives and DNR status with patients and families.
  • Document interactions and interventions based on FMCNA policies and procedures.
  • Participate in monthly quality review meetings and report on quality indicators related to patient adherence and quality of life.
  • Assess patient knowledge of kidney disease and provide tailored education to address barriers to treatment adherence.
  • Facilitate the transplant referral process and manage transplant waitlist in collaboration with the interdisciplinary team.
  • Review patient rights and responsibilities with patients and their representatives.
  • Support the clinic in scheduling to maximize patient adherence and adjustment.
  • Collaborate with Insurance Coordinators to address insurance-related issues and provide education on payment options.
  • Assist with the hiring process of new personnel if requested by management.
  • Maintain updated patient resource lists and provide training to staff on psychosocial topics.

Requirements

  • Master's degree in social work from an accredited program.
  • State-required social work license.
  • 0 - 2 years of related experience in a healthcare setting.

Nice-to-haves

  • Experience working with patients with Chronic Kidney Disease or in dialysis settings.
  • Familiarity with community resources for patient support and rehabilitation.
