About the position
The Master Social Worker (MSW) position is focused on providing psychosocial services to patients undergoing treatment at the facility, including in-center and home dialysis patients. The role emphasizes collaboration with the healthcare team to enhance patient rehabilitation, quality of life, and emotional support, particularly for those affected by Chronic Kidney Disease. This entry-level position involves patient assessment, care planning, and counseling, ensuring comprehensive care and support for patients and their families.
Responsibilities
- Assess patients' psychosocial status, strengths, and areas of need affecting rehabilitation and treatment outcomes.
- Participate in care planning with patients and the healthcare team to identify effective interventions.
- Utilize patient education programs and social work methods to address barriers and meet treatment goals.
- Monitor and provide interventions to help patients adjust to dialysis and improve their psychosocial status.
- Offer supportive counseling services to patients within the scope of clinical training and state license.
- Provide educational and goal-directed counseling for patients seeking transplant.
- Assist patients with referrals to community resources to facilitate optimal treatment outcomes.
- Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with access to these resources.
- Assess patient awareness of advance directives and facilitate discussions regarding their wishes.
Requirements
- Master's degree in Social Work (MSW) from an accredited program.
- State licensure as a social worker (LCSW or equivalent).
- Knowledge of Chronic Kidney Disease and its psychosocial impacts.
- Experience in patient assessment and care planning.
