About the position

The Master Social Worker (MSW) position at Fresenius Medical Care involves providing psychosocial services to patients undergoing dialysis treatment. This entry-level role focuses on assessing patients' psychosocial needs, collaborating with healthcare teams to enhance patient care, and supporting patients and their families in navigating the challenges associated with Chronic Kidney Disease. The MSW will engage in care planning, patient education, and quality improvement activities to ensure optimal treatment outcomes and quality of life for patients.

Responsibilities

  • Assess patients' psychosocial status and needs as part of the comprehensive patient assessment.
  • Participate in care planning with the healthcare team to identify effective interventions for rehabilitation and treatment goals.
  • Utilize patient education programs and social work methods to address barriers to treatment.
  • Provide supportive counseling services to patients and assist with transplant referrals.
  • Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with referrals.
  • Facilitate discussions regarding advance directives and DNR status with patients and families.
  • Document interactions and interventions provided to patients and families based on MSW policies.
  • Participate in monthly quality review meetings and report on quality indicators related to patient adherence and quality of life.
  • Educate patients and families about kidney disease and available support services.
  • Review patient rights and responsibilities with patients to ensure understanding.

Requirements

  • Master's degree in social work from an accredited program.
  • State-required license to practice as a social worker.
  • 0 - 2 years of related experience in a healthcare setting.

Nice-to-haves

  • Experience in dialysis or chronic illness management.
  • Familiarity with community resources for patient support.
