About the position

The Master Social Worker (MSW) at Fresenius Medical Care provides essential psychosocial services to patients undergoing dialysis treatment, both in-center and at home. This role involves collaborating with healthcare teams to enhance patient rehabilitation, quality of life, and emotional well-being, while also educating staff on the impacts of Chronic Kidney Disease. The MSW actively participates in quality improvement initiatives and ensures comprehensive care through patient assessments, counseling, and resource referrals.

Responsibilities

  • Assess patients' psychosocial status and needs as part of the comprehensive patient assessment.
  • Participate in care planning with the healthcare team to identify effective interventions for rehabilitation and treatment goals.
  • Utilize patient education programs and social work methods to address barriers and meet treatment goals.
  • Provide supportive counseling services to patients and assist with transplant referrals.
  • Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with referrals.
  • Facilitate discussions regarding advance directives and DNR status with patients and families.
  • Document interactions and interventions based on MSW policies and procedures.
  • Participate in monthly quality review meetings and report on quality indicators related to patient adherence and quality of life.
  • Educate patients and families about kidney disease and available support services.
  • Review patient rights and responsibilities with patients to ensure understanding.

Requirements

  • Master's degree in Social Work (MSW) from an accredited program.
  • State-required license to practice as a social worker.
  • 2 to 5 years of related experience in a healthcare setting.

Nice-to-haves

  • Experience in dialysis or chronic illness management.
  • Familiarity with community resources for patient support.
  • Training in psychosocial topics for staff education.
