About the position

The Master Social Worker (MSW) position at Fresenius Medical Care involves providing psychosocial services to patients undergoing dialysis treatment. This entry-level role focuses on assessing patients' psychosocial needs, collaborating with healthcare teams to enhance patient care, and supporting patients and their families in managing the emotional and psychological impacts of Chronic Kidney Disease. The MSW will also participate in quality improvement activities to ensure optimal patient outcomes and satisfaction.

Responsibilities

• Assess patients' psychosocial status and needs as part of the comprehensive patient assessment.
• Participate in care planning with the healthcare team to identify effective interventions for rehabilitation and treatment goals.
• Utilize patient education programs and social work methods to address barriers and meet treatment goals.
• Monitor and provide interventions to help patients adjust to dialysis and improve their quality of life.
• Offer supportive counseling services to patients within the scope of clinical training and state license.
• Provide educational counseling to patients seeking transplant options.
• Assist patients with referrals to community resources for optimal treatment outcomes.
• Maintain knowledge of local vocational and educational rehabilitation programs to assist patients in accessing these resources.
• Facilitate discussions regarding advance directives with patients and the healthcare team.

Requirements

• Master's degree in Social Work (MSW) from an accredited program.
• State licensure as a social worker (if applicable).
• Knowledge of social work theory and methods, particularly in relation to chronic illness and rehabilitation.

Nice-to-haves

• Experience in a healthcare setting, particularly with dialysis or chronic illness patients.
• Familiarity with community resources and rehabilitation programs.
