About the position

As a Social Worker with Fresenius Medical Care, you will provide psychosocial services for dialysis clinic patients, working collaboratively with the healthcare team to enhance patient rehabilitation and quality of life. This role involves understanding the emotional and psychological impacts of chronic kidney disease on patients and their families, while also supporting clinic staff in these areas.

Responsibilities

  • Assess patients' psychosocial status, strengths, and needs affecting rehabilitation and treatment outcomes.
  • Collaborate with patients and the healthcare team to identify effective interventions for rehabilitation and treatment goals.
  • Utilize patient education programs and social work methods to address barriers and meet treatment goals.
  • Monitor and provide interventions for patients adjusting to dialysis to achieve optimal psychosocial status.
  • Assess patient knowledge of kidney disease and identify barriers to treatment adherence.
  • Provide supportive counseling services to patients within the scope of clinical training and state license.
  • Assist patients with referrals to community resources to facilitate optimal treatment outcomes.
  • Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with referrals.
  • Assess patient awareness of advance directives and facilitate discussions with the healthcare team and family.
  • Participate in discussions regarding patient DNR status to ensure informed decision-making.
  • Report on quality indicators related to adherence and quality of life trends.
  • Provide education tailored to patients' learning styles and communication needs.
  • Facilitate the transplant referral process and collaborate on transplant waitlist management.
  • Educate patients and families about psychosocial issues related to end-stage renal disease and available support services.
  • Review patient rights and responsibilities with patients and their representatives.
  • Provide training to clinic staff on psychosocial topics as needed.

Requirements

  • Masters in Social Work
  • State required license
  • 0 - 2 years' related experience
