About the position

The Masters Social Worker in the VMFH Division Support Services is responsible for conducting social work assessments and interventions for hospitalized and emergency department patients. This role focuses on crisis intervention, patient and family support, high-risk screening, and facilitating post-acute care placements, ensuring continuity of care and addressing the unique needs of diverse patient populations.

Responsibilities

• Perform social work assessments and interventions for hospitalized and emergency department patients.
• Provide crisis intervention and brief counseling to patients and families.
• Conduct high-risk screenings and referrals for financial or resource needs.
• Facilitate family/patient representative meetings with the healthcare team as needed.
• Arrange post-acute placements for complex discharges and engage appropriate agencies or community resources.
• Advocate for patient/family self-care management and health management education.
• Identify and refer cases of abuse, neglect, or trafficking.
• Support patients and families through grief and bereavement, adoption, and surrogacy processes.
• Coordinate interdisciplinary care communication related to continuity and transitions of care planning.
• Maintain up-to-date resource lists and educate patients/families about community resources.

Requirements

• Master's degree from a school of social work accredited by the Council of Social Work Education.
• 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting.
• Licensed Advanced Social Worker (LASW:WA) or equivalent licensure.

Nice-to-haves

• Minimum 3-Year Post-MSW healthcare experience.
