About the position

The Maternity Group Home Case Manager at the Youth Service Bureau of St. Joseph County is responsible for providing intensive case management to young mothers in a transitional living program. This role focuses on helping clients develop self-sufficiency plans, facilitating life skills groups, and ensuring a supportive environment for youth facing safety and housing crises. The position emphasizes a client-centered, trauma-informed approach to empower young women and promote their well-being.

Responsibilities

• Provide intensive case management sessions using Trauma-Informed Care and Evidence-Based Practices.
• Help youth develop self-sufficiency plans and coordinate daily life skills groups.
• Offer resources on parenting and child development.
• Document case management sessions, group activities, and all interactions with clients.
• Meet with clients weekly to work on individual goals and conduct program assessments.
• Assist clients in discovering their strengths and goals.
• Enforce policies and procedures of the Transitional Living Program.
• Conduct regular room checks to ensure client safety.
• Transport residents as needed for appointments.
• Respond to calls or messages on the program phone.
• Add potential clients to the waitlist who meet program qualifications.
• Adhere to agency protocols and report to on-call staff during crises.
• Attend monthly team meetings and quarterly training sessions.

Requirements

• Bachelor's degree (preferred)
• Experience in leadership roles
• Ability to work with youth in crisis
• Strong documentation and communication skills
