About the position

The MDS Assessor plays a critical role in the healthcare team, responsible for conducting comprehensive assessments of patients to ensure appropriate care planning and compliance with regulatory standards. This position requires a strong understanding of clinical documentation improvement and the ability to work collaboratively within a hospital or managed care environment. The ideal candidate will have experience in critical care settings, such as ICU or Level II Trauma Centers, and possess a thorough knowledge of Medicare regulations.

Responsibilities

  • Conduct detailed assessments of patients' medical histories and current health status to determine care needs.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans based on assessment findings.
  • Ensure compliance with federal and state regulations regarding patient assessments and documentation standards.
  • Utilize clinical documentation improvement techniques to enhance the accuracy and completeness of patient records.
  • Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
  • Provide education and support to patients and their families regarding care options and resources available.
  • Maintain up-to-date knowledge of managed care practices, Medicare guidelines, and industry standards related to patient assessment.

Requirements

  • Bachelor's degree in Nursing, Social Work, or a related field; relevant certifications preferred.
  • Previous experience in a hospital setting, particularly in managed care or critical care environments (ICU, Level II Trauma Center).
  • Strong understanding of Medicare regulations and medical management principles.
  • Excellent clinical assessment skills with attention to detail in documentation practices.
  • Ability to work effectively within a multidisciplinary team while demonstrating strong communication skills.
  • Knowledge of primary care standards and practices is advantageous.
