About the position

The MDS Assistant - RN/LPN position at American Senior Communities focuses on providing compassionate care to residents while ensuring compliance with federal regulations and standards. The role involves conducting assessments, advocating for residents, and participating in interdisciplinary care planning to enhance the quality of life for seniors in the community.

Responsibilities

  • Provide care and compassion to residents, making a difference in their lives.
  • Complete timely, accurate, and complete assessments using RAI tools to promote individualized support and care.
  • Act as a resident advocate by facilitating family and resident participation in interdisciplinary care plan meetings.
  • Follow a cyclical schedule related to the RAI and care planning process, ensuring adherence to strict federal timelines.
  • Participate in interdisciplinary care plan meetings and conferences to ensure positive resident outcomes.

Requirements

  • Demonstrates C.A.R.E. values to residents, family members, customers, and staff.
  • Ability to work overtime as needed.
  • Graduate of an accredited School of Nursing.
  • 1-3 years of experience in MDS knowledge and data entry in the long-term care industry.
  • 2 years of professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting.
  • Knowledge of the Resident Assessment Instrument, Interdisciplinary Care planning process, and Minimum Data Set 2.0.
  • Familiarity with OBRA regulations and current state reimbursement guidelines.
  • Knowledge of current laws and regulations influencing care and services in nursing facilities.

Nice-to-haves

  • Licensed Position or better in Provider Trust License Verification
