About The Position

The MDS Assistant at Beech Grove Meadows plays a crucial role in providing compassionate care to residents in a senior living community. This position involves conducting assessments, advocating for residents, and participating in interdisciplinary care plan meetings to ensure the highest quality of care and well-being for residents. The MDS Assistant is integral to the care team, promoting individualized support and adhering to federal timelines for assessments and care planning.

Requirements

  • Demonstrates C.A.R.E. values to residents, family members, customers, and staff.
  • Ability to work overtime as needed.
  • Graduate of an accredited School of Nursing, preferably as a Registered Nurse.
  • One to three years of experience in MDS knowledge and data entry in the long-term care industry.
  • Two years of professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting.
  • Knowledge of the Resident Assessment Instrument, Interdisciplinary Care planning process, and Minimum Data Set 2.0.
  • Familiarity with OBRA regulations and current state reimbursement guidelines.
  • Knowledge of current laws and regulations influencing care and services in nursing facilities.

Responsibilities

  • Provide care and compassion to residents, making a difference in their lives.
  • Complete timely, accurate, and complete assessments using RAI tools.
  • Act as a resident advocate by facilitating family and resident participation in care plan meetings.
  • Follow a cyclical schedule related to RAI and care planning processes, ensuring adherence to federal timelines.
  • Participate in interdisciplinary care plan meetings and conferences with Social Services, family members, and residents.
