About The Position

The MDS Coordinator (LPN or RN) is responsible for leading the MDS assessments and care planning processes in a long-term healthcare and rehabilitation center. This role ensures compliance with federal and state regulations while collaborating with an interdisciplinary team to provide high-quality care to residents.

Requirements

  • State licensure as a Registered Nurse (RN) or LPN
  • 2+ years of experience in a healthcare facility or related setting
  • Strong understanding of skilled nursing standards of care
  • Excellent critical thinking and communication skills.

Responsibilities

  • Complete MDS assessments in accordance with federal and state regulations and guidelines.
  • Ensure accuracy and completeness of data.
  • Conduct comprehensive assessments of residents, including physical, psychological, and social aspects.
  • Utilize the MDS tool to capture resident status.
  • Develop and update individualized care plans based on MDS assessment data.
  • Coordinate care plan meetings with the interdisciplinary team.
  • Ensure compliance with all applicable regulations and standards, including CMS guidelines and state-specific requirements.
  • Work closely with other healthcare professionals, including physicians, therapists, social workers, and dietitians.
