About the position

The MDS Coordinator - RN is responsible for the timely and accurate completion of the MDS treatment assessment tool and for reviewing care plans to ensure compliance with high standards of care. This role involves managing Medicare/Medicaid case-mix documents to assure appropriate reimbursement for services provided within the nursing center, conducting concurrent MDS reviews, and integrating information from various services to ensure optimal patient care and reimbursement.

Responsibilities

• Complete the MDS treatment assessment tool accurately and on time.
• Review care plans and their delivery for compliance with care standards.
• Manage the overall process and tracking of Medicare/Medicaid case-mix documents.
• Conduct concurrent MDS reviews to achieve maximum allowable RUG categories.
• Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services.

Requirements

• Must be a graduate of an accredited School of Nursing with current Registered Nurse (RN) licensure.
• At least one year of long-term care nursing experience is required.
