About the position

The RN MDS Coordinator at Franciscan Villa is responsible for ensuring the accuracy of resident assessments and optimizing care plans. This role involves leading the development and completion of MDS assessments, collaborating with the interdisciplinary team, and maintaining high standards of care and regulatory compliance within the facility.

Responsibilities

• Lead and coordinate the development and completion of resident assessments (MDS) in compliance with current regulations and guidelines.
• Ensure the MDS process is completed accurately and within required timeframes.
• Collaborate with the interdisciplinary team to create personalized care plans for residents.
• Continuously monitor and document residents' progress and responses to care plans.
• Participate in the Quality Assurance and Performance Improvement (QAPI) process to enhance care quality.
• Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
• Stay informed on MDS regulations and guidelines to ensure compliance with industry standards.

Requirements

• Current RN (Registered Nurse) license
• RAC-CT (Resident Assessment Coordinator-Certified) certification is required
• Strong understanding of MDS 3.0, RAI process, and care planning
• Excellent organizational, communication, and interpersonal skills
• Ability to work independently and collaboratively within a team
• Commitment to providing high-quality care to residents
