About the position
The MDS Coordinator at Alpine Breeze Health & Wellness is responsible for managing the integrity of the facility's RAI and Medicare processes. This role is crucial in ensuring that residents receive the highest quality of care through accurate documentation and timely completion of MDS/PPS assessments. The MDS Coordinator will collaborate with the interdisciplinary team to assess resident needs and oversee care plan reviews, ensuring compliance with regulations and standards.
Responsibilities
- Collaborate with the Interdisciplinary Team to assess resident needs and provide an interdisciplinary schedule for MDS assessments and care plan reviews.
- Assist with coordination and management of daily stand-up meetings, including review of resident care and setting assessment reference dates.
- Monitor MDS and care plan documentation for all residents, ensuring documentation supports MDS coding.
- Assist the Director of Nursing (DON) or designee with identifying significant changes, physician orders, and verbal reports to ensure MDS and care plans reflect those changes.
- Ensure the completion and timeliness of the RAI Process from MDS through the completion of the plan of care.
- Report trends from completed audits to the Quality Assurance Committee.
- Maintain confidentiality of necessary information.
- Perform other duties and responsibilities as assigned.
Requirements
- Graduate of an approved Nursing program and licensed in Missouri.
- 2 years of recent experience as an MDS Coordinator.
- Current BLS/CPR certification.
- Reliable transportation.
