About the position

The MDS Coordinator is responsible for overseeing the assessment and documentation processes for residents in a healthcare facility. This role requires strong organizational skills, the ability to work under pressure, and effective communication with various departments to ensure compliance with federal and state regulations. The MDS Coordinator plays a crucial role in maintaining the quality of care and documentation for residents, while also providing support to nursing and administrative staff.

Responsibilities

  • Schedules residents for appropriate admission, annual, quarterly, Medicare assessment or change-of-condition assessments, overseeing communication to nursing and other departments.
  • Assures compliance with Federal and State regulations regarding completion/coordination of the comprehensive assessment (MDS).
  • Monitors MDS and care plan documentation on all residents, assuring that complete, orderly chronological and current documentation is in file.
  • Maintains proficiency in software.
  • Coordinates and attends regular Medicaid meetings to discuss current residents, pending residents and recipients.
  • Coordinates and attends all ITTM meetings in order to gather information, communicate changes, maintain and update the clinical records including the Care plan.
  • Performs care plan updates as needed.
  • Provides DON, Administrator and pertinent staff with the Quality Indicators Report.
  • Performs transmissions of the MDS on a timely basis.
  • Remains current with changes in the industry through attendance at outside workshops and conferences.
  • Performs Ancillary management, collection and reporting to corporate billing personnel.

Requirements

  • Licensed Nurse in good standing.
  • Geriatric nursing background.
  • Restorative care philosophy.
  • Excellent professional skill (both assessment and hands-on).
  • Good communication and organization skills.
  • Working knowledge of nursing home regulations.
  • Ability to work independently as well as a meaningful member of the team.
  • Excellent organizational skills in order to coordinate many projects simultaneously.
