About the position

The MDS Coordinator at Amelia Rehabilitation and Healthcare Center is responsible for overseeing the development and management of comprehensive care plans for residents, ensuring compliance with federal and state regulations, and maintaining high standards of care. This full-time position requires a dedicated individual who can effectively coordinate with nursing management and monitor resident status to provide quality care in a long-term care facility.

Responsibilities

  • Keep abreast of current federal and state regulations, as well as professional standards.
  • Assist with the development of comprehensive care plans for residents in coordination with the MDS.
  • Coordinate, manage, and monitor the written plan of care for each resident.
  • Assist nursing management with the coordination, management, and review of nurse's notes.
  • Monitor resident status changes to ensure appropriate nursing or clinical team involvement.
  • Assure MDS and support documentation are accurate and meet regulatory requirements.
  • Perform regular audits of documentation to assure accuracy.
  • Assist nursing management with the discharge process.
  • Perform administrative requirements, such as completing necessary forms and reports.
  • Assure that established infection control and standard precaution practices are maintained.
  • Maintain strict confidentiality regarding sensitive health information of residents.
  • Report all hazardous conditions, damaged equipment, and supply issues.
  • Maintain the comfort, privacy, and dignity of residents and interact with them respectfully.
  • Answer and respond to call lights promptly and courteously.
  • Communicate and interact effectively with residents, visitors, families, peers, and supervisors.
  • Attend and participate in departmental meetings and in-services.

Requirements

  • Current State License as a Registered Nurse (RN)
  • C.P.R. Certified

Nice-to-haves

  • One year experience as a Nurse in a long-term care setting.
  • Knowledge and training in all aspects of the MDS process.
