About the position

The MDS Coordinator at Crystal Lake Healthcare & Rehabilitation is responsible for overseeing the completion of assessments and ensuring compliance with state regulations. This role involves guiding the nursing service department in the assessment process and participating in facility audits and surveys. The position offers an opportunity to grow within a supportive and established organization.

Responsibilities

• Monitors and guides the completion of assessments in a timely manner.
• Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of the validation.
• Make written or oral reports/recommendations to the Director as necessary/required, concerning the operation of the nursing service department.
• Participates in facility surveys and audits made by authorized governmental agencies as requested by the Administrator or DON.

Requirements

• Must hold a Nursing Degree from an accredited school.
• Must have an active license as an RN.
• Experience preferred, not required.
