About the position

The MDS Coordinator at Big Oak Rehabilitation and Healthcare is responsible for coordinating the assessment and care planning processes for patients or residents. This role involves scheduling assessments, reporting patient conditions to medical professionals, and ensuring compliance with healthcare regulations through the MDS system.

Responsibilities

• Coordinate processes related to the assessment and care planning of patients or residents.
• Schedule regular assessments for patients and ask them to report on their feelings.
• Provide regular reports to doctors and other medical professionals, as well as billing departments.
• Work with the Medical Decision Support (MDS) system to ensure diseases and conditions are treated effectively.

Requirements

• Valid New Jersey Registered Nurse (RN) license
• 1-year experience as an RNAC preferred
• Extensive knowledge of MDS, PDPM, Medicare, Medicaid, and private insurance.
