About the position
The MDS Coordinator and Registered Nurse Assessment Coordinator (RNAC) at Magnolia Manor of Rock Hill is responsible for overseeing patient and resident care from admission through discharge. This role involves coordinating the Resident Assessment Instrument (RAI) process, completing the Minimum Data Set (MDS), and developing interdisciplinary care plans. The RNAC monitors patient status, identifies changes in conditions, and collaborates with the healthcare team to ensure quality care and positive outcomes.
Responsibilities
- Identifying and monitoring resident skilled services.
- Coordinating the Resident Assessment Instrument (RAI) process.
- Completing an accurate Minimum Data Set (MDS) and developing the interdisciplinary Plan of Care.
- Monitoring patient/resident status to identify changes in condition, barriers to discharge, and progress toward care goals.
- Participating in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM).
- Establishing and maintaining positive relationships with all members of the healthcare team.
Requirements
- Current, unencumbered nursing license in the state of practice.
- Minimum of 2 years of direct hands-on nursing experience.
- Minimum of 2 years of long-term care experience.
- Preferably at least 1 year of RAI experience.
- Proficiency in ICD-10 coding.
