The Med History Pharmacy Tech position at MercyOne involves critical responsibilities in gathering and documenting medication histories for patients presenting for care. This role is essential in ensuring that accurate medication information is recorded in the electronic medical record, which includes prescription medications, over-the-counter medications, vitamins, and nutritional supplements. The pharmacy technician will interview patients and their designated family members or caregivers as soon as possible after they present for care, particularly in clinical areas such as the Emergency Department, inpatient, and outpatient settings. The technician will be responsible for obtaining a comprehensive list of medications, which requires meticulous attention to detail. This includes recording the name and location of the patient's ambulatory pharmacy, the names of medications and supplements, their dosage forms, strengths, doses, and the timing of the most recent doses. The technician must also clarify uncertainties and compile a complete medication history by utilizing various sources of information, including patient and family input, pharmacy records, clinic records, long-term care facility records, and previous hospital encounters. In addition to medication history, the technician will also obtain and document an allergy history for each patient, ensuring that the nature of any allergic reactions is clearly recorded. In cases where there are unresolved uncertainties regarding a patient's medication or allergy history, the technician is expected to consult with a pharmacist to ensure accuracy and safety in patient care.
