The Med History Pharmacy Technician plays a crucial role in ensuring that patients receive safe and effective medication therapy. This position involves interviewing patients and their designated family members or caregivers as soon as they present for care in various Mercy clinical areas, including the Emergency Department, inpatient, and outpatient settings. The technician is responsible for obtaining and accurately recording a comprehensive list of medications in the electronic medical record. This includes prescription medications, over-the-counter drugs, vitamins, and nutritional supplements, along with the sources of this information. The recording process requires attention to detail, as the technician must document essential information such as the name and location of the patient's ambulatory pharmacy, the names of medications and supplements, dosage forms, strengths, doses, and the timing of the most recent doses. The technician must also clarify any uncertainties and compile a complete medication history by utilizing information from patients, family members, ambulatory pharmacy records, clinic records, long-term care facility records, and previous hospital encounters. In addition to medication history, the technician is tasked with obtaining and recording an allergy history for each patient, ensuring that the nature of any allergic reactions is clarified and documented when necessary. In cases where there are unresolved uncertainties regarding a patient's medication or allergy history, the technician is expected to consult with a pharmacist to ensure patient safety and accuracy in medication management.
