About the position

The Med History Pharmacy Technician plays a crucial role in ensuring that patients receive safe and effective medication therapy. This position involves interviewing patients and their designated family members or caregivers as soon as they present for care in various Mercy clinical areas, including the Emergency Department, inpatient, and outpatient settings. The technician is responsible for obtaining and accurately recording a comprehensive list of medications in the electronic medical record. This includes prescription medications, over-the-counter drugs, vitamins, and nutritional supplements, along with the sources of this information. The recording process requires attention to detail, as the technician must document essential information such as the name and location of the patient's ambulatory pharmacy, the names of medications and supplements, dosage forms, strengths, doses, and the timing of the most recent doses. The technician must also clarify any uncertainties and compile a complete medication history by utilizing information from patients, family members, ambulatory pharmacy records, clinic records, long-term care facility records, and previous hospital encounters. In addition to medication history, the technician is tasked with obtaining and recording an allergy history for each patient, ensuring that the nature of any allergic reactions is clarified and documented when necessary. In cases where there are unresolved uncertainties regarding a patient's medication or allergy history, the technician is expected to consult with a pharmacist to ensure patient safety and accuracy in medication management.

Responsibilities

  • Interview patients and/or their designated family/caregivers upon presentation for care.
  • Obtain and record a current list of medications in the electronic medical record.
  • Document prescription medications, over-the-counter medications, vitamins, and nutritional supplements.
  • Record the name/location of the patient's ambulatory pharmacy, medication/supplement name, dosage form, strength, dose, and timing of the most recent dose.
  • Clarify uncertainties and compile a complete medication history using various sources of information.
  • Obtain and record an allergy history for each patient, clarifying the nature of allergic reactions when necessary.
  • Consult with a pharmacist in cases of unresolved uncertainties related to medication and allergy history.

Requirements

  • High school diploma or GED required when nationally certified through NHA.
  • PCTB nationally certified technicians do NOT require HS diploma or GED.
  • Proof of completion of Mandatory Reporter abuse training specific to population served within 3 months of hire.
  • National Pharmacy Technician Certification required.
  • Minimum of 2000 hours of pharmacy technician experience required.
  • Registered as a Certified Pharmacy Technician with the Iowa Board of Pharmacy at time of hire.
