The Med History Pharmacy Tech position at Trinity Health involves interviewing patients and their caregivers to gather comprehensive medication histories and allergy information. This role is crucial in ensuring accurate medication records are maintained in the electronic medical record system, which supports patient safety and effective care delivery.

Responsibilities

• Interview patients and/or their designated family/caregivers to obtain medication histories as soon as possible after presenting for care.
• Record a current list of medications, including prescription medications, over-the-counter medications, and supplements in the electronic medical record.
• Clarify uncertainties and compile complete medication histories using various sources such as pharmacy records and previous hospital encounters.
• Obtain and record allergy histories for each patient, clarifying the nature of allergic reactions when necessary.
• Consult with a pharmacist regarding unresolved uncertainties related to a patient's medication and allergy history.

Requirements

• High school diploma or GED required when nationally certified through NHA.
• PCTB nationally certified technicians do not require a high school diploma or GED.
• Proof of completion of Mandatory Reporter abuse training specific to the population served within 3 months of hire.
• National Pharmacy Technician Certification is required.
• Minimum of 2000 hours of pharmacy technician experience is required.
• Registered as a Certified Pharmacy Technician with the Iowa Board of Pharmacy at the time of hire.
