About the position

The Med History Pharmacy Technician at MercyOne is responsible for interviewing patients and their caregivers to gather comprehensive medication and allergy histories. This role involves recording detailed information in the electronic medical record and consulting with pharmacists to resolve any uncertainties regarding patient medication and allergy histories. The position is essential in ensuring accurate medication management and patient safety in various clinical settings.

Responsibilities

• Interview patients and/or their designated family/caregivers to obtain medication histories as soon as possible after presenting for care.
• Record a current list of medications, including prescription, over-the-counter, and supplements, in the electronic medical record.
• Clarify uncertainties and compile complete medication histories using various sources of information.
• Obtain and record allergy histories for each patient, clarifying the nature of allergic reactions when necessary.
• Consult with a pharmacist in cases of unresolved uncertainties related to medication and allergy histories.

Requirements

• High school diploma or GED required when nationally certified through NHA.
• National Pharmacy Technician Certification is required.
• Minimum of 2000 hours of pharmacy technician experience is required.
• Registered as a Certified Pharmacy Technician with the Iowa Board of Pharmacy at the time of hire.
• Proof of completion of Mandatory Reporter abuse training specific to the population served within 3 months of hire.
