About the position

The Medical Assistant-Community Health Worker (MA-CHW) at Journey Health plays a vital role in providing care and administrative services as part of a multidisciplinary team. This position focuses on enhancing access to healthcare for vulnerable populations by removing barriers to care and supporting patients in achieving their health goals. The MA-CHW will work under the supervision of clinical practitioners and engage with patients both in-person and remotely to deliver high-quality, culturally sensitive care.

Responsibilities

  • Administer health screenings, diagnostic testing, and treatment procedures under the supervision of a clinical practitioner.
  • Maintain a caseload of Medi-Cal patients and provide ongoing support.
  • Facilitate patient access to social resources, including applying for Medicaid benefits and connecting with local community services.
  • Conduct telephonic and face-to-face outreach with patients to identify social determinants of health impacting their wellness.
  • Support patient care delivery by preparing laboratory specimens, performing basic laboratory tests, and conducting select clinical procedures.
  • Assist with clinical and administrative duties to optimize patient experience and care management.
  • Coordinate with patients, caregivers, and care team members to implement care plans based on patient priorities and goals.
  • Utilize coaching and motivational interviewing techniques to support patients in achieving their goals.
  • Identify barriers to achieving clinical or social outcomes and engage the care team to revise care plans as necessary.
  • Work with interdisciplinary teams to develop transition plans for patients moving from acute or post-acute settings to home.
  • Utilize technology and digital resources to monitor ongoing care activities.
  • Facilitate, track, and manage referrals to key treatments and services.
  • Document patient-related activities and interactions in electronic medical records or appropriate platforms.

Requirements

  • Community Health Worker Certification or 2,000 hours working as a CHW (or similar role) in the past 3 years, with a commitment to certification within 1 year of hire.
  • Experience navigating the healthcare system.
  • Certification in basic life support (BLS).
  • Experience providing culturally sensitive care and delivering high-quality customer service to diverse communities.
  • Clear and effective communication skills with team members, clients, and stakeholders.
  • Ability to work independently, make informed decisions, and manage tasks efficiently in a fast-paced environment.
  • Support colleagues and foster a cohesive, high-performing team.
  • Adaptability to organizational changes in a dynamic, start-up environment.
  • Proficiency in Microsoft Suite applications (Word, Excel, PowerPoint, Outlook, and Teams).
  • Familiarity with motivational interviewing, clinical documentation, and the healthcare system.
